Texas Foundations Fund Matching Grants

2016 Request Form

Please submit the form below to access your additional 2016 matching grant. TSAHC will award six $4,033 matching grants on a first-requested, first-awarded basis until all six matching grants have been awarded.

1.Public or private funds used as match must have been received on or after January 1, 2016.

2.Public or private funds used as much must be earmarked specifically for the program that your Texas Foundations Fund matching grant will support (critical repairs or supportive services).

3. Public or private funds used as match may not have been submitted as match for your original $12,100 matching grant.

4. Organizations must submit a request (and document public or private funds as match) for the full $4,033 matching grant. Partial requests will not be accepted.

5. All funding from the Texas Foundations Fund must be earmarked specifically to serve households at or below 50% AMFI.

6. All funding from the Texas Foundations Fund must be earmarked specifically to serve households with a disability or households in a rural community.

Below, please briefly describe how the matching grant will be used. Include the following in your description: 1) the number of people to be served with the amount requested, 2) the income level and funding priority of the people to be served, and 3) the specific expenses that your matching grant will cover. Note: you will be asked to submit a budget to document how your matching grant was spent in an interim report due on September 30, 2017.

Below, please provide a brief description of the public or private funds that will be matched with your Texas Foundations Fund matching grant. Include the following in your description: 1) the name of the entity providing the funding, 2) the date the funds were awarded, 3) the total amount awarded, and 4) how the funds will be used. Note: the funds must be designated for the same program that your matching grant will support.
