2017 Matching Grant Request Form

Fill out the form below to request your Texas Foundations Fund 2017 matching grant. Please note:

1. Each partner is eligible for a maximum matching grant of $13,250. To access the full matching grant, you must submit documentation of at least $13,250 in public or private funding raised.

2. To be eligible for a matching grant, the public or private funding must have been awarded after January 1, 2017 and must be designated specifically for the program that your matching grant will support. Public or private funds used as match may not have been submitted as match for your 2016 matching grant. See the 2016 Texas Foundations Fund Guidelines for additional requirements.

3. You do not need to request your matching grant all at once. You may submit up to three funding requests to access your full matching grant. However, each request must include documentation that you have raised public or private funding equal to the amount requested.

4. You have until Friday, March 30, 2018 to request your full matching grant and submit sufficient documentation of public or private funds raised. Any funds not requested by March 30, 2018 will be combined into a statewide pool and made available to other partners on a first-come, first-awarded basis until all funds are depleted.

5. All funding from the Texas Foundations Fund must be earmarked specifically to serve households at or below 50% AMFI.

6. All funding from the Texas Foundations Fund must be earmarked specifically to serve households with a disability or households in a rural community, unless your organization elects to use your matching grant to serve households that were displaced or otherwise negatively affected by a federally declared disaster.

Below, please briefly describe how the matching grant will be used. Include the following in your description:

1) the number of people to be served with the amount requested,

2) the income level and funding priority of the people to be served, and

3) the specific expenses that your matching grant will cover.

If your organization is using all or a portion of your matching grant to serve households negatively affected by a federally declared disaster, use this section to indicate what portion of your matching grant will serve those households.

Note: you will be asked to submit a budget to document how your matching grant was spent in a final report due on September 30, 2018.

Below, please provide a brief description of the public or private funds that will be matched with your Texas Foundations Fund matching grant. Include the following in your description:

1) the name of the entity providing the funding,

2) the date the funds were awarded,

3) the total amount awarded, and

4) how the funds will be used.

Note: the funds must be designated for the same program that your matching grant will support.

Below, please upload documentation of public or private funds to be matched. Examples of acceptable documents include a grant agreement, award letter, or other document that clearly indicates the amount and use of funds and date awarded.
