  • Report Documents

    The final report for the 2016 matching grant is due on October 2, 2017. The final report for the 2017 matching grant is due on September 30, 2018.  Please familiarize yourself with the sample report below so you are prepared to answer all questions.  Additionally, all households whose photos or stories are included in the report must signed the Testimonial and Photo Release Form linked below.

  • Income Documents

    Your matching grant may only be used to serve households earning at or below 50% of the area median family income.  TSAHC uses the U.S. Department of Housing and Urban Development’s Section 8 income limits to determine income eligibility for the Texas Foundations Fund program. Please use the 2016 income limits for the duration of your 2016 grant term and the 2017 income limits for the duration of your 2017 grant term.

    Below you will find the Texas Foundations Fund income guidelines and a sample income certification form.  You are not required to use TSAHC’s income certification form if you already use a form that collects the same information.

  • Disability Certification Information

    Nonprofit partners serving households with a disability must verify each household’s disability status.  Documentation from the following sources may be used to verify disability status: Social Security Administration (SSI/SSDI), U.S. Department of Veterans Affairs, Texas Department of Assistive and Rehabilitative Services, Texas Department of Aging and Disability Services, and Texas Department of Motor Vehicles (permanent disability only).  If the client does not receive benefits from one of the agencies listed above, you may also use the self-certification form linked below to verify a client’s disability status.

  • Eligible Expenses

    Partners must use 100% of their matching grant for direct costs to provide the program for which the matching grant is requested. See the link below for additional guidance.

  • Matching Grant Request Forms

    Please fill out the matching grant request form linked below to access your matching grant.  You may not submit your request form until you have signed and returned your 2017 grant agreement. 

    All matching grant requests will be processed within 5-10 business days.
