July/August 2018 Grantees: The final report is due on August 1, 2019. Quarterly reports are due on October 1, 2018; January 1, 2019; and April 1, 2019.
Additionally, all households whose photos or stories are included in the reports must have provided a signed release form. If your organization does not already have a release form, please use the release form provided below.

HEART grants may only be used to serve households earning at or below 80% of the area median family income. TSAHC and Enterprise Community Partners use the U.S. Department of Housing and Urban Development’s Section 8 income limits to determine income eligibility for the HEART program. Please use the 2018 income limits for the duration of your grant term.

Below you will find the HEART income guidelines and a sample income certification form. You are not required to use this income certification form if you already use a form that collects the same information. Please note that organizations receiving grants for critical home repairs must submit signed certification forms for all households assisted as part of their final report.

Participation in the Part I webinar is encouraged but not required. |
